

CRL and our pharmacy benefit manager (pbm), Express Scripts Inc. (ESI), are pleased to provide you with information about your prescription drug benefits. The ESI website, www.express-scripts.com, is available to all insureds with prescription drug coverage. ESI will help you with all of your pharmacy needs, whether you want to locate a pharmacy, request mail-order service, or need answers to your pharmacy questions.
CRL has contracted with ESI to provide mail-order pharmacy services. Their dispensing systems help provide quality care with the convenience of having your medications delivered directly to your home. There is no extra cost for standard shipping!
If you need to start your medication right away, ask your physician to write two prescriptions. One for a 60 or 90 day supply (whichever your plan allows) that will be sent to ESI, and one for a 30 day supply that can be filled immediately at a retail pharmacy until your mail-order prescription can be processed.
Complete a mail-order form, which you can get by calling a CRL Customer Service Representative at 1-800-253-4641 or by visiting the ESI website, www.express-scripts.com. Be sure to include your phone number in case Express Scripts needs to contact you.

You will receive your medication usually within 14 days. Included will be a mail-in refill form (with instructions), a new envelope and forms for future mail order prescriptions.
Within the last 30 days of your prescription, you can contact ESI to order your refill. This may be done by phone or at the Express Scripts website.
As a registered member at ESI’s website www.express-scripts.com, you may print out a mail-order form, order refills and check the status of your refill order. You may also be able to transfer your retail prescription to mail order. Contact ESI toll-free at 1-800-287-4526 for more information.
